How Much Do GLP-1 Programs Cost? Insurance vs. Cash-Pay

The short answer: it depends on the route

GLP-1 pricing varies enormously based on one thing: whether you go brand-name through insurance or cash-pay through a compounded program. The same goal — losing weight with semaglutide or tirzepatide — can cost a modest copay for one person and several hundred dollars a month for another.

Brand-name with insurance

Brand-name GLP-1s (Wegovy®, Zepbound®) carry high list prices, but if your plan covers them, your out-of-pocket cost can be far lower. Coverage is inconsistent: many plans require prior authorization, a documented BMI, or a diabetes diagnosis, and some exclude weight-loss drugs entirely. Manufacturer savings cards can help eligible, insured patients. A program with an insurance-navigation team is worth a lot here.

Cash-pay compounded plans

If you don't have coverage, compounded semaglutide or tirzepatide is usually the more affordable path. These plans are typically a flat monthly fee that often bundles the medication, the provider visit, and shipping together — frequently in the ballpark of $99–$300 per month depending on the medication and dose. No insurance is required.

How to compare the true cost

Don't compare headline prices — compare the all-in monthly cost. Ask what's included (medication, consults, shipping), whether the price changes as your dose increases, and whether there's a commitment. The cheapest sticker price isn't always the cheapest program once everything is added up.
